Patient Forms
Forms for check-in must be completed at our office. Please bring all other forms (FMLA, yearly physicals, sports physicals, etc.) to your appropriately scheduled appointment. Form drop-off outside of an appointment are subject to additional fees.
Insurance
We are open to a variety of different insurances. Please view the list of insurances we accept before calling to schedule an appointment. If you do not see your insurance listed, please contact our office at 419-843-3349 as we may have recently been contracted with your insurance plan's network.

Now you can book an appointment online with one of our physicians. When providing "reason for visit," please be as detailed as possible so that we can better prepare and assist you with your healthcare needs.

If you need to cancel your appointment, please notify us at least 24 hours in advance or earlier if possible, so that appointment availability is open to all patients.

Office policies
Telephone/Portal Messages
The office has been trained to answer most of your concerns. However, telephone calls and messages from the morning are usually answered by 12:45 PM and afternoon messages by 6:30 PM.
Test/Lab Results
All patients are notified of abnormal results promptly through telephone or secure web message. Please do not call the office for results unless you were specifically instructed by your physician to call back for your results.
If your labs are sent to us electronically, they will be reviewed by the physician and uploaded to your Patient Portal to view.
Referrals
We are able to care for most of your medical needs. Please make an appointment for all new medical problems. During your initial and if needed subsequent visits, your physician will determine if a consultation with another specialist is required.
For ongoing medical problems requiring care by a specialist, please call us two weeks prior to your appointment to give our office ample time to establish any needed referrals with your insurance company.
Office Charges
Payment is expected at the time of each visit. If you have coverage through an insurance plan, please bring the appropriate information with you to the office. If you do not pay your co-pay at the time of visit, you will be charged an additional $10 late fee.
No Show Fee- $25 for each instance. If you cannot make your appointment please call the office to cancel and /or reschedule your appointment.
Medication refills done through the office are subject to a refill fee of $10. To avoid this fee, please call your pharmacy to request a refill.
Medications Without Office Visits
Medications for acute illnesses are not routinely prescribed over the phone/sent electronically outside of an office visit. It is also office policy not to prescribe antibiotics unless the patient is seen in the office.
