What is the Patient Portal?
The Patient Portal is your own secure, personal website where you can schedule appointments, access labs/xrays, view visit summaries, and send secure messages to our office. It is a safe way for your to keep track of your personal health record and keep you at the forefront of your own healthcare needs.

I can't get on my Patient Portal! What do I do?

What is your policy on childhood immunizations?
In order to best protect the health of all our patients, we will not be accepting any patients who refuse CDC-recommended vaccines. In addition, if you are a current patient and choose not to vaccinate your children, we ask that you find a new doctor. Thank you for your understanding. 

Remember to always discuss what immunizations you need with your PCP and keep your immunizations up-to-date!

How do I request a refill?
Please call your pharmacy to request a refill. The only time you should request a refill directly from our office is if it is a controlled medication that requires you to pick of prescription scripts from us. We have a right to charge you a $10 processing fee if you ask for a refill directly from our office. 

Please note that all prescription (Rx) requests through the Patient Portal should be completed for in-office prescription script pick-up ONLY.

What are your office hours?
Monday-Thursday
we are open 8:30 AM-5:00 PM.  Phones turn off at 4:00 pm daily.

Friday we are open 8:30-4:30 PM. We are closed daily from 12:30-1:30 PM for lunch
We are closed on Saturday, Sunday and have no evening hours. If you need to talk with us concerning a medical emergency during the evening or weekend, call 419-843-3349 and ask for the physician on call they may offer you a telemedicine visit.


Where do you provide hospital coverage?
Springforest Family Physicians provides hospital coverage at Flower hospital. The care of inpatients in the remaining area hospitals is provided by other physicians who are specialists in hospital medicine.

I am a new patient, what do I need for my first visit?
For your New Patient visit, please arrive a few minutes early to fill out the proper paperwork.
You will need:
-A valid ID or drivers license
-A current insurance card and prescription card (If you have one) 
-A list of medications you are taking


Reminder: Physicians will not fill out any FMLA paperwork or prescribe controlled substances to new patients for the first six months.

 What Is A Wellness Exam? 

At your well exam you will not be able to discuss any new medical problems. Should your wellness exam turn into a diagnostic or problem-oriented visit we will bill accordingly.

If you need a change in any of your current medications, referrals to specialist for non-routine services (not including Mammogram and Colonoscopy), or diagnostic testing ordered - you will need to choose either the well exam or the medical visit and reschedule the other.

If a new medical problem arises in the meantime you will be asked to reschedule your well exam and we will treat you for your medical issue.

Lab work ordered as part of a routine exam may only include a lipid profile and a metabolic profile.  Additional testing will not be covered as routine.   If you require additional testing due to maintenance medications (such as liver profile, thyroid testing etc…) these can be ordered at a well exam but cannot be billed as routine.

​Most insurance companies will cover a well exam at 100% but not all. It is your responsibility to know if a well exam is covered with your particular coverage. If you have any questions please contact your insurance carrier to see exactly what is covered under your wellness benefits.
